Composition:

Each film-coated tablet contains Metformin Hydrochloride USP 500 mg

🔬 Pharmacology

• Decreases hepatic glucose production
• Reduces intestinal glucose absorption
• Improves insulin sensitivity by increasing peripheral glucose uptake
• Does not cause hypoglycemia or hyperinsulinemia

✅ Indications

Adjunct to diet and exercise to improve glycemic control in adults and children (10+ years) with type 2 diabetes mellitus.

💊 Dosage & Administration

Always individualized based on patient’s response & tolerance. Must be taken with meals.

⚠️ Special Populations

• Pregnancy: Category B – Use only if clearly needed. Insulin is preferred.
• Nursing Mothers: May cause hypoglycemia in infants. Consider stopping drug or nursing.
• Elderly: Use lowest effective dose due to reduced renal function.

❌ Contraindications

• Renal disease or dysfunction
• Known hypersensitivity to Metformin
• Metabolic acidosis, including diabetic ketoacidosis

⚠️ Precautions

• Monitor renal function closely
• Risk of lactic acidosis especially in renal impairment
• Avoid use if serum creatinine >1.5 mg/dL (males), >1.4 mg/dL (females)

⚠️ Warnings

• Lactic acidosis is rare but potentially fatal
• Gradual dose escalation minimizes GI side effects

😖 Adverse Effects

• Diarrhea, nausea, vomiting
• Abdominal discomfort, indigestion
• Headache, flatulence, asthenia

🔄 Drug Interactions

• ↑ Absorption with: Nifedipine
• No effect on: Cimetidine
• May interact with: Diuretics, corticosteroids, phenothiazines, estrogens, oral contraceptives, phenytoin, calcium blockers, and cationic drugs (e.g., digoxin, morphine, quinidine)
